Participation Agreement & Liability Waiver
TIGER ESPORTS ATX — PARTICIPATION AGREEMENT, LIABILITY WAIVER, AND PARENTAL CONSENT

By checking the agreement box and completing checkout, I confirm that I am the parent or legal guardian of the student named in this registration order, and I agree to the following on my own behalf and on behalf of my student.
PROGRAM DESCRIPTION. Tiger Esports ATX operates after-school esports and social-emotional learning (SEL) clubs, a video game design club, and summer day camps for middle school students (the "Program"). Activities are non-contact and classroom-based, consisting of supervised video game play on Program equipment, SEL curriculum activities, and related instruction, held on school district campuses.
CONSENT TO PARTICIPATE. I give my permission for my student to participate in all Program activities, and I confirm that my student is physically and medically able to participate.
ACKNOWLEDGMENT AND ASSUMPTION OF RISK. I understand that participation carries risks that cannot be entirely eliminated, including risks ordinarily present on school premises (such as slips, trips, and falls), risks associated with extended screen use and seated computer activity, and risks of interaction among students. I knowingly and voluntarily accept these risks on behalf of my student.
RELEASE AND WAIVER. To the fullest extent permitted by law, I, on my own behalf, release, waive, and discharge Collaborative Edge Youth Program LLC (DBA Tiger Esports ATX), its owners, employees, contractors, and agents, and the host school districts (the "Released Parties"), from any and all claims, demands, or causes of action arising out of or relating to my student's participation in the Program, except to the extent caused by gross negligence or willful misconduct. I agree not to sue the Released Parties for such released claims, and I agree to indemnify the Released Parties against claims brought by third parties arising from my or my student's violation of Program rules.
MEDICAL AUTHORIZATION. In the event of an injury or medical emergency, I authorize Program staff to secure emergency medical treatment for my student, including first aid, calling 911, and transport by emergency medical services, if I or my emergency contacts cannot be reached promptly. I understand I am responsible for the costs of any such treatment. Program staff will make reasonable efforts to contact me immediately.
BEHAVIOR AND TECHNOLOGY USE. My student agrees to follow Program rules and staff instructions, to treat other participants respectfully, and to use Program equipment and accounts only as directed. The Program may remove a student from a session for behavior that endangers or disrupts others; repeated violations may result in removal from the Program without refund of the current session.
STUDENT INFORMATION AND PHOTO CONSENT. Guardians and caretakers should email collaborativeedgeyouth@gmail.com to provide any important information about their child that is not collected by the registration form, including emergency contacts, authorized pickup persons, allergies, and medical conditions. The Program occasionally photographs activities for parent communications and Program promotion, and by agreeing to this document I consent to that use of my student's photo. If I do not want my student photographed or shown in Program materials, I will email collaborativeedgeyouth@gmail.com to opt out, and the Program will honor that request.
GENERAL TERMS. This agreement is governed by Texas law. If any provision is found unenforceable, the remaining provisions continue in effect. This agreement applies to the registration purchased in this order and remains in effect for the current Program year, including summer camp sessions, unless revoked by me in writing to collaborativeedgeyouth@gmail.com.
